Clinical Social Worker II

We are seeking a highly skilled and experienced Clinical Social Worker II to join our team at The Royal Mental Health Centre. As a key member of our interdisciplinary team, you will provide evidence-based social work services to individuals with severe and complex concurrent substance use, mental health, and physical health disorders.
Key Responsibilities- Provide comprehensive biopsychosocial assessments, consultations, and treatment plans for clients with complex needs.
- Collaborate with hospital and community partners to ensure seamless care coordination and service delivery.
- Advocate on behalf of clients and their families to access necessary resources and services.
- Develop and implement crisis intervention plans and respond effectively to crisis situations.
- Engage clients, families, and community supports in clinical interventions and risk reduction strategies.
- Assess clients' social determinants of health and provide assistance as needed.
- Utilize evidence-informed therapeutic approaches, including motivational interviewing, cognitive behavioral therapy, and dialectical behavior therapy.
- Lead group-based therapy modules and provide education to clients and their families.
- Document client information accurately and maintain confidentiality.
- Participate in peer consultation, education, and supervision of Social Work students.
- Stay current with hospital policies, procedures, and legislation.
- Provide educational presentations to community groups and conferences.
- Flexibility in work schedule, including day and evening hours.
- Liaise with community service providers and work collaboratively to achieve client goals.
- Master's degree in Social Work from an accredited institution.
- Registration and good standing with the Ontario College of Social Workers and Social Service Workers.
- Experience as a clinical social worker in a mental health and substance use treatment setting.
- Training and experience working with individuals with severe and complex substance use and concurrent mental health disorders.
- Training and experience in individual, family, and group work.
- Knowledge and experience with harm reduction interventions, including safer injection practices and overdose prevention and response.
- Knowledge and experience working with individuals with high-risk drug use and blood-borne infections and STIs.
- Experience identifying signs and symptoms of alcohol withdrawal or intoxication, and opioid withdrawal or intoxication.
- Ability to work effectively in an interdisciplinary team and collaborate with hospital and community partners.
- High level of knowledge, skill, and confidence in using electronic health record systems for clinical documentation and navigation.
- Bilingual (French/English) is mandatory.
