About the Role

We are seeking a highly skilled and compassionate Social Worker to join our team at FunctionAbility Rehabilitation Services. As a Social Worker, you will play a vital role in providing support and guidance to individuals and families in need.

Key Responsibilities
  • Conduct Comprehensive Assessments

Conduct thorough psychosocial assessments to identify the needs and goals of clients, and develop personalized plans to address their challenges.

Develop and Implement Intervention Plans

Work with clients to develop and implement intervention plans that promote social and emotional well-being, and provide ongoing support and guidance to ensure successful outcomes.

Provide Counseling and Therapy Services

Offer counseling and therapy services to help clients resolve social and personal problems, and provide education and support to promote healthy coping mechanisms.

Collaborate with Interdisciplinary Teams

Work collaboratively with healthcare professionals, social workers, and other stakeholders to provide comprehensive care and support to clients.

Advocate for Clients

Advocate on behalf of clients to ensure their needs are met, and provide support and guidance to help them navigate complex systems and services.

Develop and Implement Prevention and Intervention Programs

Develop and implement prevention and intervention programs to meet the needs of clients and communities, and provide education and support to promote healthy behaviors and outcomes.

Requirements
  • Education

A Bachelor's degree in Social Work or a related field is required.

Experience

At least 1 year of experience in a social work or related field is preferred.

Skills

Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and stakeholders.

Language

Fluency in English is required.
