In the role of Occupational Therapist, you will evaluate and identify patient care requirements, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Key Responsibilities:

Conduct comprehensive assessments of clients across various environments, including private residences, educational institutions, senior living facilities, and long-term care establishments.

• Develop realistic, measurable, and time-sensitive treatment objectives and formulate individualized treatment plans.

• Ensure that all occupational therapy interventions are initiated with the informed consent of the client or their authorized representative, in compliance with relevant healthcare legislation.

• Provide direct therapy and educational support to informal caregivers, educational staff, and other healthcare professionals involved in the patient's care.

• Engage in case discussions as necessary and collaborate with other healthcare team members to promote a holistic approach to patient care.

• Perform follow-up assessments to evaluate patient progress and treatment goals, communicating findings to the Care Coordinator and other team members to ensure continuity of care and identify additional resources as needed.

• Collaborate with patients, families, and Care Coordinators to plan for the conclusion of services once treatment goals are met or when services are no longer required.

• Assess and recommend assistive devices and adaptations necessary to meet treatment objectives, adhering to the standards of the relevant assistive devices program.

• Maintain accurate documentation and patient records in accordance with professional standards and organizational policies.

• Serve as a resource for colleagues, service providers, and community partners by offering educational programs as requested.

• Actively participate in occupational therapy team meetings and organizational staff meetings.

• Stay current with professional knowledge and skills through ongoing education and training.

• Prioritize occupational therapy services in line with established waiting list management policies.

Qualifications:

A degree or diploma in Occupational Therapy and a valid registration with the College of Occupational Therapists of Ontario.

• A minimum of two years of relevant professional experience in an Occupational Therapy capacity.

• Experience in a community-based setting is advantageous.

• Current CPR-C certification.

Preferred Skills:

Knowledge:

• Familiarity with the healthcare system and the role of community care services.

• Understanding of operational principles, quality assurance, and performance management within healthcare.

• Awareness of relevant healthcare legislation and organizational policies.

• Insight into community partners and contracted service providers.

Skills and Abilities:

• Excellent interpersonal skills to engage with diverse patient populations.

• Ability to navigate challenging conversations effectively.

• Proven capability to work both independently and as part of a team.

• Strong communication skills to maintain relationships with community professionals and organizations.

• Exceptional customer service skills.

• Effective listening and facilitation skills to ensure optimal service delivery to patients and their families.

• Competence in delivering information in various settings, including individual consultations and team meetings.

• Strong organizational skills to manage multiple patient cases and documentation requirements.

• Proficiency in computer applications, including Microsoft Office Suite.

• A valid driver's license and access to a vehicle are required.

Bilingualism in English and French is an asset. A recent Vulnerable Sector Check is required.

Work Schedule

Monday to Friday – 8:30 AM to 4:30 PM (35 hours/week)
