In the role of Occupational Therapist, you will evaluate and identify patient care requirements, facilitate access to community resources, and collaborate with patients, their families, and other healthcare professionals.

Key Responsibilities:

  • Conduct thorough assessments of clients across various settings, including private residences, educational institutions, and long-term care facilities, utilizing both objective and subjective data.
  • Develop realistic, measurable treatment objectives and create comprehensive treatment plans.
  • Ensure that all occupational therapy interventions are initiated with the informed consent of the client or their designated decision-maker, in compliance with relevant legislation.
  • Provide direct therapy and educational support to informal caregivers and other professionals involved in the patient's care.
  • Participate in case discussions as necessary and collaborate with the healthcare team to promote a holistic approach to patient care.
  • Reassess patients to monitor progress and adjust treatment goals, ensuring continuity of care and identifying additional resources as needed.
  • Collaborate with patients, families, and care coordinators to plan for the conclusion of services when goals are met or when services are no longer required.
  • Make recommendations for assistive devices and adaptations necessary to meet treatment objectives, adhering to the guidelines of relevant programs.
  • Maintain accurate patient records and documentation in accordance with professional standards and organizational policies.
  • Serve as a resource for staff and community partners by providing educational programs as requested.
  • Actively engage in team meetings and contribute to organizational discussions.
  • Commit to ongoing professional development and maintaining current knowledge and skills.
  • Prioritize service delivery in line with organizational policies regarding waitlist management.

Qualifications:

  • A degree or diploma in Occupational Therapy with a valid registration from the relevant regulatory body.
  • A minimum of two years of relevant professional experience in an Occupational Therapy capacity.
  • Experience in a community-based setting is advantageous.
  • Current CPR certification.

Preferred Skills:

  • Familiarity with the healthcare system and the role of community care services.
  • Understanding of operational, quality, and performance management principles.
  • Knowledge of applicable healthcare legislation and organizational policies.
  • Strong interpersonal skills to effectively engage with diverse patient populations.
  • Ability to navigate challenging conversations with professionalism.
  • Proven capability to work both independently and collaboratively within a team.
  • Excellent communication skills for establishing connections with various stakeholders.
  • Strong customer service orientation and effective listening skills.
  • Proficiency in planning and organizing to manage multiple patient needs.
  • Competence in completing necessary documentation accurately.
  • Proficient in computer applications, including Microsoft Office.
  • A valid driver's license and access to a vehicle are required.
  • Bilingualism in English and French is an asset.
  • A recent Vulnerable Sector Check is required.

Work Schedule

This position follows a Monday to Friday schedule, offering a hybrid work model that includes both in-office and remote work options.
