In the role of Occupational Therapist, you will evaluate and identify patient care requirements, facilitate access to community resources, and collaborate with patients, their families, and other health care professionals.

Key Responsibilities:

Conduct thorough assessments of clients across various environments, including private residences, educational institutions, long-term care facilities, and group homes, utilizing both objective and subjective data.

• Set realistic, measurable, and time-sensitive treatment objectives and create tailored treatment plans.

• Ensure that all occupational therapy interventions are initiated with informed consent from the client or their designated decision-maker, adhering to relevant legislation.

• Provide direct therapeutic interventions and educational support to informal caregivers, educational staff, and other health care professionals involved in the patient's care.

• Participate in multidisciplinary case discussions as necessary and collaborate with other health care team members to promote a holistic approach to patient care.

• Conduct follow-up assessments to monitor patient progress and adjust treatment goals, reporting findings to the Care Coordinator and other team members to ensure continuity of care.

• Collaborate with patients, families, and Care Coordinators to plan for the conclusion of services once treatment goals are met or if the patient no longer requires assistance.

• Assess and recommend adaptive aids, assistive devices, and necessary equipment to support treatment objectives, while respecting the guidelines of relevant programs.

• Maintain accurate patient documentation and records in compliance with professional standards and organizational policies.

• Serve as a resource for colleagues, service providers, and community organizations by offering educational programs as needed.

• Actively engage in team meetings and contribute to organizational discussions.

• Stay current with professional knowledge and skills through ongoing education and training.

• Prioritize service delivery in line with established waiting list management policies.

Qualifications Required:

A degree or diploma in Occupational Therapy and a valid registration with the College of Occupational Therapists of Ontario.

• A minimum of two years of relevant professional experience in an Occupational Therapy capacity.

• Experience in a community-based setting is advantageous.

• Current CPR-C certification.

Preferred Skills:

Knowledge:

• Familiarity with the health care system and the operational role of community care services.

• Understanding of quality, risk, and performance management principles.

• Awareness of relevant health care legislation and organizational policies.

• Insight into community partners and service providers.

Skills and Abilities:

• Excellent interpersonal skills to engage with diverse patient populations.

• Proficient in managing challenging conversations effectively.

• Ability to work collaboratively in a team environment as well as independently.

• Strong communication skills to maintain relationships with various stakeholders.

• Demonstrated customer service excellence.

• Effective listening and facilitation skills to ensure optimal service delivery.

• Capable of presenting information in various formats, including individual consultations and group settings.

• Strong organizational skills to manage multiple patient cases and documentation.

• Proficient in using computer software and Microsoft Office applications.

• A valid driver's license and access to a vehicle are necessary.

Bilingual proficiency in English and French is an asset. A recent Vulnerable Sector Check is required.

Work Schedule

Standard hours are Monday to Friday, 8:30 AM to 4:30 PM (35 hours per week).
