Residential Rehabilitation Worker (Counselor)

Job Overview

Under the supervision of the Small Options Homes Director, the Residential Rehabilitation Worker (RRW/Counselor) is expected to operate within the Philosophy and Principles, Rules of Work, and Policy Manual of Balsam Care.

Qualifications

  • Education or experience in a Human Services field or a Community College Diploma
  • At least one year of experience or a combination of education, training, and relevant work experience
  • Must be capable of lifting 50 lbs.

Key Responsibilities

The Counselor is accountable for the following duties, along with any additional responsibilities as needed:

  • Ensure the physical and mental well-being of all clients is preserved and not jeopardized by other clients or staff.
  • Monitor, document, and communicate any changes in physical and mental health, and accompany clients to specific appointments when necessary.
  • Promote regular personal hygiene practices, including bathing, tooth brushing, and grooming.
  • Facilitate healthy dietary habits and encourage clients to maintain nutritious diets.
  • Administer, document, and/or supervise medications as per policy and provide basic first aid.
  • Ensure clients have access to recreational and leisure activities.
  • Assist clients in participating in recreational and leisure pursuits as required.
  • Support referrals to suitable community resources and liaise with those resources on behalf of the client.
  • Maintain appropriate attire and ensure client comfort and clothing allowances are managed properly.
  • Encourage and support positive interactions and relationships both within and outside the home.
  • Assist clients in fulfilling their individual spiritual needs.

Client Care and Programming

Teach and supervise essential skills necessary for achieving optimal independence, including but not limited to independent living skills, life skills, household management, and socialization.

Identify client needs and collaborate with the program team to develop, implement, and evaluate programs that address these needs.

Support and assist clients with their activities and responsibilities.

Monitor and document client activities and interactions within their living environment.

Maintain accurate and up-to-date files, records, reports, and daily logs in a professional manner.

Facilitate menu planning, grocery shopping, and meal preparation to ensure nutritional requirements are met.

Conduct formal assessments of clients' skills and progress.

Stay informed about community resources and their availability.

Provide support to staff in other homes as required.

Communication

Assist in fostering a collaborative team approach in daily decision-making within the home.

Communicate effectively with the House Coordinator, staff team, clients, families, municipal workers, healthcare providers, advocates, neighbors, and community service employees.

Be responsible for assessments, program plans, monthly progress reports, daily logs, medication charts, incident reports, and maintaining up-to-date client files.

Review recent entries in each client's progress notes at the start of each shift.

Participate in meetings as required.

Promote a welcoming atmosphere for visitors.

Provide orientation and training to new employees, volunteers, and students.

Assess and provide orientation to prospective clients.

Safety and Maintenance

Make informed decisions and take appropriate actions during crises or for crisis prevention.

Adhere to policies, procedures, and uphold standards at all times.

Ensure the home is maintained in a clean, tidy, and well-kept condition.

Be knowledgeable about and implement Emergency Evacuation Plans and other safety measures as necessary.

Recommend maintenance and replacement needs to the House Coordinator.

Ensure the security of records and the house upon departure.

General Responsibilities

Act as a role model for clients at all times.

Attend staff development sessions as required.

Be responsible for the operation of the home and the overall safety and well-being of clients in the absence of the House Coordinator.

Fulfill the role of teacher and role model for clients.

Work flexible shifts as necessary.
