We are seeking a highly skilled and compassionate Residential Rehabilitation Worker to join our team at Health Association Nova Scotia. As a key member of our care team, you will be responsible for providing exceptional support and care to individuals with varying abilities in a residential rehabilitation setting.
Key Responsibilities- Ensure the physical and mental health of individuals served is maintained and not put at risk by other persons or staff.
- Observe, document, and communicate changes in physical and mental health, and accompany individuals to appointments as needed.
- Ensure regular personal hygiene habits are followed, and healthy diets are provided and encouraged.
- Administer medications and perform simple first aid as directed by policy.
- Provide opportunities for recreation and leisure activities, and accompany individuals as required.
- Assist with referrals to community resources, and liaise with those resources on behalf of the individual.
- Ensure proper attire and comfort, and maintain clothing allowances.
- Stimulate and support interactions and interpersonal relations within and outside of the home.
- Encourage and support individuals in fulfilling their spiritual needs.
- Teach and supervise skills necessary for optimum independence, including independent living skills, life skills, and household management.
- Identify individual needs and develop, implement, and evaluate programs to meet those needs in cooperation with the program team.
- Support and assist with individual activities and responsibilities.
- Monitor and document individual activities and interactions within their home.
- Keep files, records, reports, and daily logs up to date and in a legible professional manner.
- Carry out menu planning, grocery shopping, and meal preparation to ensure nutritional needs are met.
- Complete formal assessments of individual skills and progress.
- Develop an interdependent team approach in day-to-day decisions in the home.
- Communicate with the House Coordinator, staff team, individuals, family, municipal workers, doctors, advocates, neighbors, landlord, and community service employees.
- Responsible for assessments, program plans, monthly progress reports, daily logs, medication charts, and appointments.
- Read recent entries in individual progress notes at the beginning of shift.
- Attend and participate in meetings as required.
- Make decisions and take action in crisis situations or crisis prevention.
- Follow policies, procedures, and uphold standards.
- Ensure the home is clean, tidy, and well-kept at all times.
- Be familiar with and implement Emergency Evacuation Plans and other safety measures as needed.
- Recommend maintenance and replacement needs to the House Coordinator.
- Grade 12 or education/experience in a Human Services field or Community College Diploma.
- One-year experience or a combination of education, training, and related work experience is an asset.
- Experience supporting individuals with varying abilities is an asset.
- The ability to work a variety of shifts, including days, evenings, weekends, overnights, and sleep shifts.
- Valid Nova Scotia Driver's License.
- The ability to lift pounds.
- Excellent verbal and written communication skills.
- Patient and empathetic.
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Ability to make independent decisions and good judgment in times of crisis.
