Horizon Health Network is seeking an exceptional and forward-thinking individual for the role of Interim Director of Primary Health Care. This pivotal position requires a leader with a proven track record in managing large teams and possessing robust skills in strategy, leadership, stakeholder engagement, operational management, quality assurance, and patient care.

Candidate Profile:
The ideal candidate will have:
- A Master’s degree in Health Administration, Health Care, Social Work, or a related field, or an equivalent combination of education and experience.
- A minimum of five years in a management role overseeing an inter-professional health care team, or equivalent leadership experience.
- A solid understanding of the Community Health Care model.
- Knowledge of the standards governing regulated health care professionals.
- Experience supervising diverse disciplines and programs.
- Comprehensive knowledge of management principles, including financial and human resource management.
- Experience in program development, evaluation, and quality improvement.
- Strong analytical, problem-solving, planning, decision-making, leadership, organizational, and change management skills.
- Excellent communication skills, both written and verbal.
- A strong advocate for teamwork and collaboration.
- Proficiency in word processing, spreadsheets, and email applications.
- Advanced proficiency in English, with the ability to communicate in French being a valuable asset.
Key Responsibilities:
The Director of Primary Health Care will spearhead efforts to enhance the quality of primary health care services in the region. This role involves mentoring, coaching, and supervising teams to develop and implement innovative solutions aimed at improving health outcomes.
Reporting to the Executive Regional Director of Primary Health Care, the Director will be responsible for crafting and executing the organization’s primary health care strategy, while engaging with providers, partners, and stakeholders. The role also includes advancing initiatives aligned with Horizon's Community portfolio and the provincial health plan.
The Director will oversee the delivery of quality primary health care through health centers, outreach services, and regional programs. This includes ongoing program development, service delivery, and evaluation within the framework of primary health care philosophy and Horizon's strategic objectives.
Additionally, the Director will manage all aspects of staff administration for their teams and collaborate with external agencies to ensure effective service delivery.
