Horizon Health Network is dedicated to providing exceptional healthcare services in New Brunswick. We are currently seeking a dedicated and self-motivated Indigenous Patient Navigator to enhance our support for Indigenous clients and their families.

Role Overview

The Indigenous Patient Navigator will serve as a vital resource and liaison for Indigenous individuals and their families, healthcare professionals, and community members. This role is essential in ensuring that care is culturally appropriate and centered around the needs of the client. By collaborating with patients, families, and healthcare providers, the navigator will facilitate access to necessary services and assist with discharge planning.

Key Responsibilities
  • Utilize a Two-Eyed Seeing approach to promote well-being and culturally safe care for Indigenous clients.
  • Engage with Indigenous clients and their families to ensure a positive experience within healthcare facilities.
  • Advocate for clients, enhancing their health literacy and empowering them to make informed health decisions.
  • Facilitate access to cultural, spiritual, and linguistic resources to support traditional healing practices.
  • Provide navigation support and consultation to Indigenous clients and their healthcare teams, while educating staff on Indigenous community resources.
  • Participate in case conferences and discharge planning for Indigenous clients transitioning to home or long-term care.
  • Collaborate with community stakeholders to ensure smooth transitions during and after hospitalization.
  • Maintain a comprehensive understanding of Indigenous community resources and develop written materials for clients and healthcare teams.
  • Conduct educational sessions for staff and community partners regarding Indigenous health programming and needs.
  • Assist clients in navigating the complaints process to ensure proper follow-up.
  • Compile daily and monthly statistical reports for internal review.
Qualifications
  • 1-3 years of relevant experience in Indigenous health or social services.
  • Strong advocacy skills and experience in client support.
  • Ability to manage priorities in a dynamic environment.
  • Proficiency in Microsoft Office applications.
  • Exceptional mediation and conflict resolution skills.
  • Strong interpersonal skills to build trusting relationships.
  • Knowledge of Indigenous healing practices and person-centered care.
  • Familiarity with local Indigenous demographics, cultures, and traditions.
  • Ability to communicate in Wolastoqey or Mi'kmaq is an asset.
  • Understanding of cultural safety and the Two-Eyed Seeing framework in healthcare.
  • Awareness of the impacts of colonization and systemic racism on Indigenous health.
  • Ability to work independently and make informed decisions in complex situations.
  • Written and spoken proficiency in English is required.
  • Willingness to travel as needed.
  • Valid driver's license and access to a vehicle.
Educational Background
  • Undergraduate degree in health or social sciences.
  • Additional certifications in health-related fields are preferred.
