Our Edmonton Zone Continuing Care team provides services through five main program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care.

Job Summary:

We are seeking a Registered Nurse to join our CHOICE team as a Case Manager. In this role, you will work collaboratively with the multi-disciplinary team to provide case management and deliver therapeutic nursing services within professional practice standards. You will be responsible for assessing client needs, coordinating resources, establishing goals for client delivery, and evaluating outcomes.

Key Responsibilities:

  • Provide case management services to clients in the community
  • Deliver therapeutic nursing services within professional practice standards
  • Assess client needs and develop care plans
  • Coordinate resources and services to meet client needs
  • Evaluate outcomes and make adjustments to care plans as needed

Requirements:

  • Completion of an accredited nursing education program
  • Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA)
  • Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP)
  • Preferred qualifications: Baccalaureate degree in nursing, palliative care experience, previous Continuing Care and/or geriatric experience, experience with Meditech, Connect Care, and the RAI Home Care assessment tool
