The Edmonton Zone Continuing Care sector offers a variety of services across five primary program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are seeking a Registered Nurse to become an integral part of our Facility Living team, providing expert support and consultative services to facility operators and residents in a continuing care environment.

As a proactive member of the Continuing Care team, the Registered Nurse exemplifies strong collaboration skills and undertakes responsibilities that include assessing client needs, delivering appropriate therapeutic nursing services in line with professional practice standards, coordinating resources, setting client care goals, and evaluating outcomes. This role also encompasses case management for clients, along with the direct provision of care that adheres to established clinical standards, ensuring it is evidence-based and centered around the client.

The Registered Nurse serves as a consultant within the team, offering clinical expertise and guidance to fellow team members regarding their cases. In partnership with clients and case managers, the direct RN formulates care plans to address unmet nursing needs, utilizing suitable internal and external resources through comprehensive nursing assessments and interventions.

Description:

In the capacity of a Registered Nurse (RN), you will deliver a diverse range of nursing services to patients, families, communities, and populations, while prioritizing their safety and well-being. Your role will involve employing nursing processes through critical thinking, problem-solving, and decision-making, as well as educating, counseling, and advocating for patients and their families. You will take on a leadership position in promoting an integrated and holistic approach to patient care, health promotion, and maintenance.

You will ensure the delivery of safe, high-quality, patient- and family-centered care, reflecting the shared vision and values of AHS. Required Qualifications:

- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- If you have not practiced as a Registered Nurse within the last five (5) years, completion of specific courses is required.

Additional Required Qualifications:

- Exceptional organizational and time management abilities.
- Strong communication and assessment skills to collaborate effectively with healthcare team members, clients, and their families in a dynamic environment.
- Excellent interpersonal skills.
- Proven critical thinking and decision-making capabilities.
- Physically capable of performing all assigned tasks.
- The successful candidate will operate independently in various client settings, which may include diverse home environments.

Preferred Qualifications:

- A baccalaureate degree in nursing is preferred; however, relevant skills and experience in community nursing will be considered.
- Experience in Palliative Care.
- Familiarity with Meditech and Connect Care systems.
- A minimum of two years of current nursing practice, with at least one year in a Continuing Care setting.
- Experience managing complex clients with acute, chronic, and end-of-life conditions.
