We are seeking a highly skilled and dedicated Registered Nurse to join our Home Living team at Alberta Health Services. As a key member of our multidisciplinary team, you will provide professional support services to clients of all ages and stages of life in their homes.
Key Responsibilities- Assess client needs and deliver appropriate therapeutic nursing services within professional practice standards.
- Coordinate resources and establish goals for client delivery and evaluation of outcomes.
- Provide case management and direct client care that meets established clinical standards and is evidence-based and client-centred.
- Act as a consultant on the team, providing clinical expertise and direction to other team members regarding client cases.
- Participate in staff development and mentoring student clinical experiences.
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- Completion of the following free courses: CRNA Medication Management - Chapters and the NextGenU Health Assessment Resources Certification (if not practiced as a Registered Nurse within the last five years).
- A baccalaureate degree in nursing.
- Palliative care experience.
- Previous Continuing Care and/or geriatric experience.
- Experience with Meditech, Connect Care and the RAI Home Care assessment tool.
- 2 years of current Case Management Experience in Continuing Care.
