Your Opportunity:

As a vital member of the multidisciplinary team at Alberta Health Services, the Nurse Case Manager in the Chronic Kidney Disease Clinic plays a crucial role in delivering care to patients diagnosed with stages 4 and 5 Chronic Kidney Disease (CKD). This position involves comprehensive responsibilities including the assessment, planning, implementation, and evaluation of patient care strategies. Utilizing nursing expertise alongside established treatment protocols, the Nurse Case Manager formulates individualized patient care plans grounded in a case management framework.

The Nurse Case Manager diligently reviews laboratory results, patient symptoms, and other diagnostic information to evaluate the effectiveness of care provided, ensuring timely communication of findings with nephrologists. A key aspect of this role is to educate patients and their families, which includes developing tailored learning assessments based on the patient's medical history, current health status, and personal beliefs regarding health and treatment options.

Key Responsibilities:

  • Deliver formal education on Renal Replacement Therapy and Conservative Kidney Management.
  • Engage in program development and quality improvement initiatives.
  • Utilize critical thinking and problem-solving skills to advocate for patients and their families.
  • Support a holistic approach to patient care and health promotion.

Qualifications:

Candidates must have completed an accredited nursing education program and hold an active or eligible registration with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification is also required. For those who have not practiced as a Registered Nurse in the past five years, completion of specific courses in medication management and health assessment is mandatory.

Preferred Qualifications:

A Bachelor of Science in Nursing (BScN) is preferred, along with prior experience in a CKD clinic, chronic disease management, and home dialysis programs. Familiarity with Connect Care and the Nephrology Information System (NIS) is advantageous.
