The Edmonton Zone Continuing Care division offers services across five primary program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are seeking a Registered Nurse to become an integral part of our Home Living team, delivering professional support services to clients of diverse ages and life stages.
As a proactive member of the Continuing Care team, the Registered Nurse will exemplify strong collaborative skills and will be responsible for a variety of tasks, including assessing client needs, providing suitable therapeutic nursing services that adhere to professional practice standards, coordinating resources, setting goals for client care, and evaluating the effectiveness of interventions. This role also encompasses case management responsibilities and the direct provision of care that aligns with established clinical standards, ensuring that care is evidence-based and centered around the client.
The Registered Nurse will serve as a consultant within the team, offering clinical guidance and expertise to fellow team members regarding their cases. In partnership with clients and case managers, the RN will develop care plans that address unmet nursing needs, utilizing appropriate internal and external resources through comprehensive nursing assessments and interventions.
This position is crucial to the Short-Term Intervention Team (STIT), which provides triage and clinic support, as well as community visits that necessitate specialized skills in areas such as cardiac care, IV therapy, and wound management.
Description:
In the capacity of a Registered Nurse (RN), you will deliver a broad spectrum of nursing services to patients, families, communities, and populations, while ensuring their safety and well-being. Your role will involve applying nursing processes through critical thinking, problem-solving, and decision-making, as well as educating, counseling, and advocating for patients and their families.
You will assume a leadership position in fostering an integrated and holistic approach to patient care, health promotion, and maintenance. Your commitment will be to provide safe, high-quality, patient-centered care that reflects the shared vision and values of AHS.
Required Qualifications:
- Completion of an accredited nursing education program.
- Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA).
- Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
- If you have not practiced as a Registered Nurse within the last five (5) years, completion of specific free courses is required.
Additional Required Qualifications:
- Exceptional organizational and time management skills.
- Strong communication and assessment abilities to collaborate effectively with healthcare team members, clients, and their families in a dynamic environment.
- Excellent interpersonal skills.
- Proven critical thinking and decision-making capabilities.
- Physically capable of performing all assigned tasks.
The successful candidate will operate independently in various client home settings, which may include congregate living environments, single-family homes, condominiums, and walk-up apartments. Therefore, the ability to adapt to different settings and environmental conditions is essential, as is the capacity to lift and transport medical equipment weighing approximately 25-30 lbs.
Preferred Qualifications:
- A baccalaureate degree in nursing is preferred; however, relevant skills and experience in community nursing will be considered.
- Experience in Palliative Care.
- Familiarity with Meditech and Connect Care systems.
- A minimum of two years of current nursing practice, with at least one year in a Continuing Care setting.
- Experience managing complex clients with acute, chronic, and end-of-life conditions.
- Proficiency in cardiac care, IV therapy, and wound management.
