Edmonton, Alberta, Canada Alberta Health Services Full time

Your Opportunity:

Alberta Health Services (AHS) is committed to enhancing community health, enabling individuals to maintain their well-being and independence within their own homes and neighborhoods. The Edmonton Zone Continuing Care offers a range of services across five primary program areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We are currently seeking a Registered Nurse to become an integral part of our Facility Living team, providing expert support and consultative services to facility operators and residents within a continuing care environment.

As a proactive member of the Continuing Care team, the Registered Nurse will exemplify strong collaborative skills and undertake various responsibilities, including assessing patient needs, delivering appropriate therapeutic nursing services in accordance with professional practice standards, coordinating resources, setting client care goals, and evaluating health outcomes. This role also encompasses case management and the direct provision of patient care that adheres to established clinical standards, ensuring a client-centered and evidence-based approach. The Registered Nurse will serve as a consultant within the team, offering clinical expertise and guidance to fellow team members regarding their cases. Collaborating closely with clients and case managers, the Registered Nurse will develop care plans that address unmet nursing needs, utilizing both internal and external resources through comprehensive nursing assessments and interventions.

Description:

In the capacity of a Registered Nurse (RN), you will deliver a diverse array of nursing services to patients, families, communities, and populations, while taking essential measures to ensure their safety and well-being. Your role will involve employing nursing processes, utilizing critical thinking, problem-solving, and decision-making skills, as well as teaching, counseling, and advocating for patients and their families. You will take on a leadership role in promoting an integrated and holistic approach to patient care, health promotion, and maintenance.

Your responsibilities will include providing safe, high-quality, patient- and family-centered care, reflecting the shared vision and values of AHS. Please note that provisional permit holders will receive a specified pay rate, which will be adjusted upon registration as a Registered Nurse in accordance with applicable agreements.

Required Qualifications:

To qualify, you must have completed an accredited nursing education program and be actively registered or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification is also required. If you have not practiced as a Registered Nurse within the last five years, completion of specific free courses is mandatory.

Additional Required Qualifications:

Successful candidates will demonstrate excellent organizational and time management skills, alongside strong communication and assessment abilities to work effectively with healthcare team members, clients, and their families in a dynamic environment. Interpersonal skills, critical thinking, and decision-making capabilities are essential. Candidates must be physically capable of performing all assigned tasks, including working independently in various client home settings. This may involve lifting and carrying medical equipment weighing approximately 25-30 lbs. During the interview process, candidates may be required to demonstrate their computer proficiency through a hands-on assessment.

Preferred Qualifications:

A baccalaureate degree in nursing is preferred; however, relevant skills and experience in community nursing practice will be considered. Experience in Palliative Care, Meditech, and Connect Care is advantageous. Candidates should have a minimum of two years of current nursing practice, with at least one year in a Continuing Care setting, and experience managing complex clients with acute, chronic, and end-of-life conditions.
