Position Overview

As a vital member of our team, the Community Support Worker will play a crucial role in delivering care and assistance to individuals residing in supported living environments. Your focus will be on fostering independence and empowering those we serve to lead fulfilling lives while adhering to high standards of care, health, and safety. Your compassionate nature and ability to collaborate effectively within a multidisciplinary team will be key to achieving success in this role.

Key Responsibilities

  • Uphold the rights and preferences of supported individuals, advocating on their behalf when necessary.
  • Engage meaningfully with the individuals receiving support.
  • Encourage the individuals to actively participate as members of their community.
  • Contribute to the creation, execution, and documentation of Personal Service Plans.
  • Build and maintain positive relationships with family members, professionals, colleagues, and community members.
  • Assist with daily living activities and medication management.
  • Demonstrate a solid understanding of behavior management techniques, including addressing mental health needs.
  • Document daily activities and personal health information (PHI) of the individuals supported in our case management system.
  • Collaborate with supported individuals to ensure their living environment is clean and well-kept, including meal preparation and light housekeeping tasks.
Qualifications

  • Completion of Grade 12 education.
  • Possession of a Community Support Worker Diploma or equivalent qualification.
  • A minimum of 2 years of experience in community social services or healthcare.
  • Current certifications in Emergency First Aid/CPR and Food Safety.
  • Completion of a behavior management training course.
  • Strong verbal and written communication skills for effective interaction with team members, professionals, families, and community members.
  • Proficiency in non-verbal communication.
  • Possession of a Restricted Class 4 Driver's License (for Full-Time roles).
  • A satisfactory Driver's Abstract.
  • Clearance of criminal record checks for working with children and vulnerable adults.
  • Recent TB Test results.
  • Moderate computer skills.
  • Eligibility to work in Canada.
