Job Summary

We are seeking a highly motivated and compassionate Community Support Specialist to join our team at STRIVE Support Services. In this role, you will work closely with clients in a group home setting, providing support and guidance to help them achieve their goals and improve their overall well-being.

Key Responsibilities
  • Prepare and maintain accurate reports and case histories for clients
  • Assess clients' needs and eligibility for specific services, and develop personalized rehabilitation programs
  • Conduct follow-up assessments and evaluations to measure the effectiveness of treatment programs
  • Develop and implement the delivery of specific services within the community, including housekeeping and food preparation activities
  • Supervise activities of clients living in group homes and half-way houses, and provide support and guidance as needed
  • Liaise with other social services agencies and healthcare providers involved with clients
Requirements
  • Mandt System training
  • Non-violent Crisis Intervention (NVCI) Certificate
  • First Aid Certificate
  • CPR Certificate
  • Criminal record check
  • Valid driver's licence and own vehicle
Working Conditions

The successful candidate will be able to work independently and as part of a team, with a strong attention to detail and excellent interpersonal and communication skills. They will also be able to work in a fast-paced environment and adapt to changing circumstances as needed.
