As a Community Support Specialist, you will work with clients in a group home setting, providing support and guidance to help them achieve their goals and improve their overall well-being.
Key Responsibilities:
- Prepare reports and case histories to document client progress and needs
- Assess client needs and eligibility for specific services
- Plan and implement rehabilitation programs with offenders
- Conduct follow-up assessments to evaluate program effectiveness
- Develop and deliver specific services within the community
- Supervise activities of clients living in group homes and half-way houses
- Liaise with other social services agencies and healthcare providers
Requirements:
- Mandt System training
- Non-violent Crisis Intervention (NVCI) Certificate
- First Aid Certificate
- CPR Certificate
- Criminal record check
- Valid driver's licence
