Job Summary

We are seeking a skilled Community Support Specialist to join our team at STRIVE Support Services. As a key member of our team, you will play a vital role in providing support services to individuals in need.

Key Responsibilities
  • Prepare reports and case histories to document client progress and needs
  • Assess clients' needs and eligibility for specific services, and develop rehabilitation programs to address these needs
  • Conduct follow-up assessments to evaluate the effectiveness of treatment programs
  • Develop and implement the delivery of specific services within the community, including housekeeping and food preparation
  • Supervise activities of clients living in group homes and half-way houses, and implement substance abuse treatment programs
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support for clients
Requirements
  • Mandt System training and Non-violent Crisis Intervention (NVCI) Certificate
  • First Aid and CPR Certificates
  • Criminal record check and valid driver's licence
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
