Job Title: Community Worker

We are seeking a highly motivated and compassionate Community Worker to join our team at STRIVE Support Services. As a Community Worker, you will play a vital role in supporting individuals in our group homes and half-way houses, helping them to achieve their goals and improve their overall well-being.

Key Responsibilities:
  • Prepare reports and case histories to track client progress and identify areas for improvement.
  • Assess clients' needs and eligibility for specific services, and develop personalized rehabilitation programs.
  • Conduct follow-up assessments to evaluate the effectiveness of treatment programs and make adjustments as needed.
  • Develop and implement the delivery of specific services within the community, including housekeeping and food preparation activities.
  • Supervise activities of clients living in group homes and half-way houses, and provide support and guidance as needed.
  • Liaise with other social services agencies and healthcare providers to ensure comprehensive support for clients.
  • Assist in evaluating the effectiveness of treatment programs and make recommendations for improvement.
Requirements:
  • Mandt System training and Non-violent Crisis Intervention (NVCI) Certificate.
  • First Aid and CPR Certificates.
  • Criminal record check.
  • Valid driver's licence and own vehicle.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Attention to detail and ability to prioritize tasks.
