Job Title: Integrated System Navigator - Income Support and Stability

Job Summary:

The Parry Sound District Social Services Administration Board is seeking a highly skilled and compassionate Integrated System Navigator to join our team in the Income Support and Stability department. As a key member of our team, you will play a vital role in supporting individuals and families by creating client-led service plans and connecting them with community resources.

Key Responsibilities:

  • Develop and implement client-led service plans to support individuals and families in finding and using community resources.
  • Provide assertive case management and coordination with Community Service Providers to ensure the best possible outcomes for participants.
  • Explore and obtain additional stability support and financial resources for participants.
  • Collaborate with internal and external stakeholders to ensure a comprehensive understanding of participant needs and develop effective solutions.
  • Stay up-to-date with relevant legislation, Board policies, and best practices in the field of social services.

Requirements:

  • Two-year College Diploma in a field related to Human Services.
  • Membership with the Ontario College of Social Workers and Social Service Workers is an asset.
  • Minimum two years of experience in a social service environment, preferably in a role related to homelessness or mental health and addictions.
  • Knowledge of computer-managed documentation and related input and output documents.
  • Valid class G driver's license, access to a reliable vehicle, and ability to travel throughout the region.
