Integrated Care Lead

The University Health Network is seeking a highly skilled and compassionate Integrated Care Lead to join our team. As a critical member of the Integrated Care Program, you will be responsible for providing leadership and coordination of patient care across various settings, ensuring seamless transitions and exceptional outcomes.
Key Responsibilities- Act as the primary contact for patients, care partners, and clinical teams, facilitating communication and coordination of care.
- Develop and implement patient care plans, ensuring comprehensive and coordinated care across acute and community settings.
- Provide mentorship and coaching to care team members, promoting best practices and knowledge transfer.
- Collaborate with inter-professional teams, family members, and community providers to ensure ongoing assessment and adjustment of patient care.
- Coordinate care transitions, ensuring effective exchange of knowledge and resources to enhance patient self-management and quality of care.
- Monitor and evaluate patient and system-level objectives, making recommendations for improvement and innovation.
- Lead ongoing innovation and optimization efforts, synthesizing data to inform care planning and decision-making.
- Undergraduate or graduate degree in a related health profession.
- Current membership in good standing with applicable regulatory health professional body in Ontario.
- Five years of clinical experience, with a focus on community health and acute care settings.
- Demonstrated experience and competence in patient navigation, case management, and coordination of care.
- Excellent interpersonal, communication, and leadership skills, with the ability to work independently and collaboratively.
- Cultural sensitivity awareness and skills to inform and best plan and facilitate coordination of patient care.
