We are seeking a skilled Integrated System Navigator to provide holistic support to individuals and families in the Income Support and Stability department. As a key member of our team, you will work closely with clients to create personalized service plans, identifying specific goals to enhance life stability and well-being.
Key Responsibilities- Client Assessment and Planning: Conduct initial and ongoing assessments to determine eligibility, employability, housing needs, and required stability supports for participants.
- Case Management and Coordination: Provide assertive case management and coordination with Community Service Providers to ensure participants receive necessary support and resources.
- Application and Documentation Support: Assist participants in completing applications, forms, and documentation to explore and obtain additional stability support and financial resources.
- Education: Two-year College Diploma in a field related to Human Services.
- Experience: Minimum two years of experience in a social service environment, with knowledge of case management, trauma-informed care, and working with complex needs.
- Skills: Strong understanding of cultural safety and cultural competency, knowledge of computer-managed documentation, and a valid class G driver's license.
