Integrated Human Services Navigator
Income Support and Stability

Employment Type: Full-time Contract, 35 hours per week, Monday to Friday

Compensation: $27.39/hour (in accordance with Collective Agreement), plus 4% vacation pay

We are in search of an Integrated Human Services Navigator to adopt a comprehensive approach to integrated human services within the Income Support and Stability department. Reporting to the Supervisor of Income Support and Stability, the Navigator will assist individuals and families by formulating a client-centered service plan that identifies specific service objectives aimed at enhancing life stability and overall well-being.

Qualifications (partial list)
  1. Two-year College Diploma in a relevant field related to Human Services.
  2. Membership with the Ontario College of Social Workers and Social Service Workers is advantageous.
  3. A minimum of two years of experience in a social service setting.
  4. Experience in case management, trauma-informed care, and working with individuals facing complex challenges such as homelessness or mental health and addiction issues is preferred.
  5. Familiarity with computer-managed documentation and related input/output documents; knowledge of SAMS is desirable.
  6. A strong understanding of cultural safety and cultural competency for Indigenous and racialized populations.
  7. Knowledge of the ethical standards in human services, with experience in conducting interviews and assisting individuals and families in accessing community resources.
  8. A valid class G driver's license, access to a reliable vehicle, and the ability to travel as required.
Responsibilities
  1. Conduct initial and ongoing assessments of eligibility, employability, housing needs, and necessary stability supports for participants.
  2. Provide assertive case management and coordination with Community Service Providers for participants, adhering to directives, Board Policies, and legislation.
  3. Assist participants and families in completing applications, forms, and documentation to explore and secure additional stability support and financial resources.
