Job Summary

The Scope Navigator, Community Integration Specialist plays a critical role in leading the system navigation function of the SCOPE program, collaborating with the interprofessional team and interdisciplinary supports in the North York community.

Key Responsibilities
  • Provide leadership in planning, facilitating change, evaluating, and coordinating with all stakeholders to ensure seamless care for patients.
  • Serve as a single point of access for Primary Care Providers (PCPs) to access appropriate resources, triaging, assessing, planning, and communicating with the interdisciplinary team.
  • Develop individualized care plans with internal and external stakeholders to facilitate seamless care for PCPs and their patients, particularly for complex patients requiring care coordination across multiple sectors.
  • Support the hospital's academic mandate to transform care through research and innovation, leveraging innovation in virtual care and technology.
Requirements
  • Regulated Allied health professional in good standing with the respected registered college.
  • Minimum 3 years of experience in the acute care or community setting.
  • Computer proficient and able to work remotely.
  • Relationship management with PCPs, the interdisciplinary team, specialists, community support service providers, North York based clinics, and their administrative assistants.
  • Strong knowledge of hospital/community resources.
  • Ability to problem solve and critically think through complex cases requiring multidisciplinary coordination.
  • 3 years of experience in caring for persons living with chronic disease and mental health disorders preferred.
  • Bilingualism in French/English is an asset.

This is a Full Time Temporary Position of 18 months with 8 hour shifts, Days shifts in the Community Integration Department. We offer a highly competitive total compensation package which may include benefits, pension, vacation pay, or pay in lieu of benefits, when applicable.
