We are seeking highly skilled and compassionate System Navigators to join our team at the Parry Sound District Social Services Administration Board. As a System Navigator, you will play a vital role in providing holistic support to individuals and families in need, helping them achieve stability and well-being.
Key Responsibilities- Develop and implement client-led service plans to address specific needs and goals
- Conduct thorough assessments to identify areas of support and create personalized plans
- Collaborate with community service providers to ensure seamless support and coordination
- Provide assertive case management and coordination to help individuals and families access necessary resources
- Initiate and assist with applications, forms, and documentation to explore additional stability support and financial resources
- Two-year College Diploma in a field related to Human Services
- Membership with the Ontario College of Social Workers and Social Service Workers is an asset
- Minimum two years of experience in a social service environment
- Experience in case management, trauma-informed care, and working with complex needs such as homelessness or mental health and addictions is an asset
- Knowledge of computer-managed documentation and related input and output documents
- Knowledge of the code of ethics of human services, with experience in conducting interviews and assisting individuals and families in finding and using community resources
- Valid class G driver's license, access to a reliable vehicle, and ability to travel throughout the district as required
