Join Vancouver Coastal Health (VCH) as a Home Support Supervisor
Vancouver Coastal Health is seeking a Licensed Practical Nurse (LPN) with supervisory experience to lead the Home Support team at Central & West Community Health Centre. This role is pivotal in ensuring quality care for our clients.
Responsibilities of the Home Support Supervisor include:
Collaborating with clients and their families to set safe and achievable goals, while assessing, planning, coordinating, and delivering comprehensive care in partnership with an interprofessional team. Evaluating service eligibility, facilitating the creation and adjustment of care plans, and providing direct care management to empower clients in navigating community services.Required Qualifications
Education & Experience
Completion of a recognized Licensed Practical Nurse program. Active registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM). A minimum of two years of recent experience with geriatric clients and adults with chronic and acute conditions, including at least one year in a supervisory capacity. Possession of a valid BC Class 5 Driver's License. Willingness to travel locally using a personal vehicle.Knowledge & Skills
Proven skills in medication administration as per the LPN Scope of Practice. In-depth understanding of various healthcare disciplines and their roles in client care. Familiarity with the standards set by the BCCNM for LPNs. Extensive knowledge of care for Medical, Surgical, and Palliative clients. Demonstrated leadership abilities to guide and mentor team members, fostering a collaborative environment. Strong problem-solving skills and resourcefulness. Ability to tailor communication to diverse audiences within a complex community context. Effective oral and written communication skills with colleagues, healthcare professionals, clients, and families. Capacity to work independently and collaboratively with others. Ability to manage multiple priorities and work under pressure. Proficiency in CPR techniques. Competence in word processing, database management, and operating computerized client information systems. Physical capability to fulfill the responsibilities of the position.Additional Information
Work hours, including days off and work locations, may vary based on operational needs. Successful candidates may need to undergo a Criminal Records Review Check.
As per current Public Health Orders, all employees of Vancouver Coastal Health must be fully vaccinated for COVID-19 or have received the most recent updated vaccine. Proof of vaccination will be required.
