Licensed Practical Nurse Hemodialysis Education Specialist

We are seeking a highly skilled Licensed Practical Nurse to join our team at Providence Health Care. As a key member of our hemodialysis unit, you will provide exceptional patient care and work collaboratively with our interdisciplinary team to deliver high-quality services.
Key Responsibilities- Assess, plan, implement, evaluate, and document patient care during pre-dialysis, dialysis, and post-dialysis periods.
- Perform patient admission duties, including assessing condition, individual patient needs, and planning nursing care in collaboration with the RN and other members of the nephrology team.
- Collaborate with the RN and other members of the nephrology team to provide nursing duties related to hemodialysis, such as weighing patients, cannulation of established fistula or graft, and continuous heparin regulation.
- Monitor patients, recognize changes, and communicate with RN any problems, changes, or unusual signs or symptoms of the patient while in the dialysis setting.
- Document observations, treatment, and other patient care activities during the pre, intra, and post-dialysis periods.
- Work collaboratively with RN regarding patient assignment to assess changes in patients, both physical and emotional, and determine the appropriate level of care required.
- Maintain knowledge of relevant equipment used for the dialysis process, theory, and mechanics of dialysis, to recognize problems with machinery and inform appropriate staff members.
- Attend various meetings, such as staff meetings, quality improvement, and patient rounds, to provide and obtain information and feedback about patients and patient care.
- Assist with orientation of new staff by explaining and demonstrating work procedures and proper use of equipment and supplies.
- Maintain and enhance knowledge of nephrology issues and trends through educational sessions, in-services, literature reviews, and workshops as required.
- Provide a safe environment for patients and staff by following safety instructions and reporting any unsafe situations to the Supervisor.
- Perform related clerical duties, such as answering the telephone, taking and relaying messages, and assisting in maintaining standardized records.
- Graduation from a recognized program for Practical Nurses with the full range of current licensed practical nursing competencies.
- Successful completion of a recognized Dialysis Program for Licensed Practical Nurses.
- One (1) year recent related experience working as a Licensed Practical Nurse in acute care, or an equivalent combination of education, training, and experience.
- Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives.
- Current basic cardiac life support certification.
