Job Title: Family Practitioner, Community Transitional Care Program

Job Summary:

We are seeking a Family Practitioner to join our team at the Community Transitional Care Program. As a Family Practitioner, you will provide primary health care services to clients with mental health and substance use issues, working closely with other healthcare professionals to ensure comprehensive care.

Key Responsibilities:

  • Provide primary health care services to clients with mental health and substance use issues
  • Work collaboratively with other healthcare professionals to ensure comprehensive care
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed
  • Participate in multidisciplinary teams to share knowledge and best practices

Requirements:

  • Eligibility for provisional or full licensure with the College of Physicians and Surgeons of British Columbia
  • Certification with the College of Family Physicians or equivalent
  • Experience working with patients with mental health and substance use issues
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize multiple tasks
