Family Practitioner

We are seeking a Family Practitioner to join our team at the Community Transitional Care Program. As a Family Practitioner, you will play a crucial role in providing primary health care and mental health support to clients as they transition from the Forensic Psychiatric Hospital to the community.
About the Role:- Provide high-quality primary health care to clients with complex mental health needs
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Conduct assessments and provide treatment for clients with substance use disorders
- Work closely with community partners to ensure seamless transitions and reintegration into the community
BC Mental Health & Substance Use Services (BCMHSUS) is a provincial leader in providing specialized mental health and substance use services. Our team is dedicated to delivering high-quality care and support to individuals and families affected by mental illness and addiction.
Requirements:- Eligibility for provisional or full licensure with CPSBC
- Certification with the College of Family Physicians, or equivalent
- Experience in working with patients with complex mental health needs and substance use disorders
