Family Practitioner - Community Transitional Care Program

BC Mental Health & Substance Use Services (BCMHSUS) is seeking a Family Practitioner to join our team in the Community Transitional Care program. As a key member of our healthcare team, you will provide primary care services to clients transitioning from the Forensic Psychiatric Hospital to the community.

Key Responsibilities:

  • Provide high-quality primary care services to clients with complex psychiatric and substance use needs
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans
  • Contribute to the development of policies and procedures to improve healthcare services for forensic patients

Requirements:

  • Eligibility for provisional or full licensure with CPSBC
  • Certification with the College of Family Physicians, or equivalent
  • Experience working with patients with complex psychiatric and substance use needs
