Family Practitioner, Community Transitional Care Program

We are seeking a skilled Family Practitioner to join our team at the Community Transitional Care Program. As a Family Practitioner, you will play a vital role in providing primary health care and mental health support to clients as they transition from the Forensic Psychiatric Hospital to the community.
About the Role:- Provide high-quality primary health care to clients with complex mental health and substance use needs.
- Work collaboratively with a multidisciplinary team to develop and implement individualized care plans.
- Conduct regular assessments and provide ongoing support to clients as they reintegrate into the community.
- Collaborate with community partners to ensure seamless transitions and continuity of care.
BC Mental Health & Substance Use Services (BCMHSUS) is a provincial leader in providing specialized mental health and substance use services. Our team is dedicated to delivering high-quality care and support to individuals and families affected by mental illness and addiction.
Requirements:- Eligibility for provisional or full licensure with CPSBC.
- Certification with the College of Family Physicians, or equivalent.
- Experience working with patients with complex mental health and substance use needs.
