Family Practitioner

Job Summary:
We are seeking a Family Practitioner to join our team at the Community Transitional Care Program. As a Family Practitioner, you will provide primary health care services to clients transitioning from the Forensic Psychiatric Hospital to the community. This is an exciting opportunity to work with a diverse patient population and make a meaningful difference in their lives.
Key Responsibilities:
- Provide primary health care services to clients in the Community Transitional Care Program
- Work collaboratively with a multidisciplinary team to provide comprehensive care to clients
- Develop and implement individualized care plans to meet the unique needs of each client
- Provide education and support to clients and their families to promote health and well-being
- Participate in quality improvement initiatives to enhance the delivery of care
Requirements:
- Eligibility for provisional or full licensure with the College of Physicians and Surgeons of British Columbia
- Certification with the College of Family Physicians of Canada or equivalent
- Experience working with patients with complex psychiatric and substance use disorders
- Strong communication and interpersonal skills
- Ability to work collaboratively as part of a multidisciplinary team
