Job Title: Family Practitioner, Community Transitional Care Program

Job Summary:

We are seeking a Family Practitioner to join our team at the Community Transitional Care Program. As a Family Practitioner, you will provide primary health care services to clients transitioning from the Forensic Psychiatric Hospital to the community. This is an exciting opportunity to work with a diverse patient population and make a meaningful difference in their lives.

Key Responsibilities:

  • Provide primary health care services to clients in the Community Transitional Care Program
  • Work collaboratively with a multidisciplinary team to provide comprehensive care to clients
  • Develop and implement individualized care plans to meet the unique needs of each client
  • Provide education and support to clients and their families to promote health and well-being
  • Participate in quality improvement initiatives to enhance the delivery of care

Requirements:

  • Eligibility for provisional or full licensure with the College of Physicians and Surgeons of British Columbia
  • Certification with the College of Family Physicians of Canada or equivalent
  • Experience working with patients with complex psychiatric and substance use disorders
  • Strong communication and interpersonal skills
  • Ability to work collaboratively as part of a multidisciplinary team
