Position Overview
Employment Type: Temporary Full-Time
Department: Spinal Cord Rehabilitation Program
Reporting Structure: Reports to Manager
Work Schedule: Days
Compensation: Competitive hourly rate
Key Responsibilities
- Conduct thorough assessments of patients to determine service needs and develop appropriate treatment plans.
- Collaborate with patients, families, and interdisciplinary teams to implement effective intervention strategies aimed at fostering positive behavioral and environmental changes.
- Organize and facilitate family and team meetings to enhance communication and support.
- Continuously evaluate and adjust intervention plans based on patient progress and feedback.
- Connect with community agencies to provide comprehensive support for patients.
- Complete necessary documentation for Alternate Level of Care (ALC) facilities, ensuring seamless communication among healthcare providers.
- Deliver immediate services that benefit both the hospital and the broader community.
- Provide clinical education and mentorship to Social Work students and other healthcare professionals.
- Engage in relevant research initiatives and projects to advance the field of social work.
- Participate in activities that promote the effective operation of the Social Work department.
- Fulfill additional duties as required within the scope of the position.
- Maintain knowledge of the Health Care Consent Act and be familiar with Advance Care Planning.
Required Qualifications
- Master's Degree in Social Work or equivalent is mandatory.
- Registration with the Ontario College of Social Workers and Social Service Workers is essential.
- Membership in the Ontario Association of Social Workers is preferred.
- A minimum of 2-3 years of experience working within an interdisciplinary team in a healthcare environment is preferred.
- Strong verbal and written communication skills are required.
- Excellent interpersonal, organizational, and time management abilities are essential.
- Proven problem-solving skills are necessary.
- Research experience or education is considered an asset.
- Experience in student education and supervision is advantageous.
- Familiarity with policies and legislation related to social welfare is beneficial.
- Proficiency in computer applications is preferred.
- Knowledge of a second language is an asset.
