Position Overview

Employment Type: Temporary Full-Time
Department: Spinal Cord Rehabilitation Program
Reporting Structure: Reports to Manager
Work Schedule: Days
Compensation: Competitive hourly rate

Key Responsibilities

  • Conduct thorough assessments of patients to determine service needs and develop appropriate treatment plans.
  • Collaborate with patients, families, and interdisciplinary teams to implement effective intervention strategies aimed at fostering positive behavioral and environmental changes.
  • Organize and facilitate family and team meetings to enhance communication and support.
  • Continuously evaluate and adjust intervention plans based on patient progress and feedback.
  • Connect with community agencies to provide comprehensive support for patients.
  • Complete necessary documentation for Alternate Level of Care (ALC) facilities, ensuring seamless communication among healthcare providers.
  • Deliver immediate services that benefit both the hospital and the broader community.
  • Provide clinical education and mentorship to Social Work students and other healthcare professionals.
  • Engage in relevant research initiatives and projects to advance the field of social work.
  • Participate in activities that promote the effective operation of the Social Work department.
  • Fulfill additional duties as required within the scope of the position.
  • Maintain knowledge of the Health Care Consent Act and be familiar with Advance Care Planning.

Required Qualifications

  • Master's Degree in Social Work or equivalent is mandatory.
  • Registration with the Ontario College of Social Workers and Social Service Workers is essential.
  • Membership in the Ontario Association of Social Workers is preferred.
  • A minimum of 2-3 years of experience working within an interdisciplinary team in a healthcare environment is preferred.
  • Strong verbal and written communication skills are required.
  • Excellent interpersonal, organizational, and time management abilities are essential.
  • Proven problem-solving skills are necessary.
  • Research experience or education is considered an asset.
  • Experience in student education and supervision is advantageous.
  • Familiarity with policies and legislation related to social welfare is beneficial.
  • Proficiency in computer applications is preferred.
  • Knowledge of a second language is an asset.
