Position Overview

Employment Type: Temporary Full-Time
Department: Spinal Cord Rehabilitation Program
Reporting Structure: Reports to Manager
Work Schedule: Days
Compensation: Competitive hourly rate

Key Responsibilities

  • Conduct comprehensive assessments of patients to determine service needs and develop tailored treatment recommendations.
  • Collaborate with patients, families, and the interprofessional team to implement effective intervention strategies aimed at fostering positive behavioral and environmental changes.
  • Facilitate family and team meetings to enhance communication and care coordination.
  • Regularly reassess and adjust intervention plans as necessary to meet evolving patient needs.
  • Engage with community agencies to ensure comprehensive support for patients.
  • Complete necessary applications for Alternate Level of Care (ALC) facilities and maintain communication with relevant team members and community organizations.
  • Deliver immediate services that benefit both the hospital and the broader community.
  • Provide clinical education and supervision to Social Work students and medical trainees.
  • Participate in research initiatives and related projects to contribute to the advancement of social work practices.
  • Support the operational efficiency of the Social Work professional group through various administrative tasks.
  • Stay informed about the Health Care Consent Act and advance care planning protocols.

Required Qualifications

  • A Master's Degree in Social Work or an equivalent qualification.
  • Registration with the Ontario College of Social Workers and Social Service Workers is mandatory.
  • Preferred membership in the Ontario Association of Social Workers.
  • A minimum of 2-3 years of experience working within an interprofessional team in a healthcare environment is preferred.
  • Strong verbal and written communication skills.
  • Excellent interpersonal, organizational, and time management abilities.
  • Proven problem-solving skills.
  • Research experience or education is considered an asset.
  • Experience in student education and supervision is advantageous.
  • Familiarity with policies and legislation related to social welfare services.
  • Proficiency in computer applications is preferred.
  • Knowledge of a second language is an asset.
