Reporting to the Program Manager, the Intake & Community Liaison will be responsible for assessing eligibility to one of SHIP’s residential programs which provides short term crisis support to clients who are experiencing a mental health and/or addiction crisis and who are in conflict with the criminal justice system. The program is intended to divert persons from incarceration, the justice system, and/or unnecessary hospitalization by providing them with voluntary community crisis accommodation and support.
The individual will be responsible for coordinating services, conducting intake assessments, and liaising with community partners to provide linkages and supports to clients.
**Duties and Responsibilities**:
- Using standardized assessment tools within a community setting
- Developing and maintaining a community resource network and partnerships within the community to facilitate client service and access
- Liaising between the program and community partners
- Maintaining communication within the program and between program locations
- Referring to partnering organizations and providing linkages to community supports
- Maintaining standards of documentation to ensure that the provision of services is effective and efficient
- Providing time-limited short-term case management when required
- Service co-ordination, follow up and advocacy as needed
- Facilitation and development of groups within the community as per client need
- Completing evaluations and feedback surveys with applicants and/or their families
- Providing comprehensive, holistic, person and family centred access to services
- Responsible for client/resident safety
- Maintaining statistical data on client services
- Ensure appropriate records are kept and make sure that statistical information is maintained as per agency standards
- Staff adheres to agency Health & Safety Program and the Client Safety Framework in the promotion of client safety
- Perform additional duties as required/delegated by Management
**Qualifications**:
- A degree/diploma in social work, psychology, nursing, health sciences or a related field and at least four to five years assessment and/or case management experience within the field of Mental Health Services
- Demonstrated knowledge of serious mental illness, concurrent disorders and/or dual diagnosis, and experience with intake services preferred
- Excellent assessment, communication and administrative skills
- Excellent time management and organizational skills
- Excellent individual and group facilitation skills
- Experience working in a community setting essential
- Dedicated team player, with strong communication skills
- Able to work with mínimal direction or supervision
- Highly flexible, with solid interpersonal skills that allow one to work effectively in a diverse working environment
- A valid Ontario Class “G” Driver’s license and a vehicle with adequate insurance coverage is required
- Strong working knowledge of Microsoft Office software (e.g. Excel, Word, etc.)
- CPR, CPI and ASIST certification
**Working Conditions and Physical Requirements**:
- Day shifts Monday - Friday, with the ability to work excess/overtime hours where required
- Able to sit for prolonged periods of time
- Able to answer and communicate through telephone calls and general computer use
- Able to lift up to 20lbs (products, documents, etc.)
- Able to react and respond quickly under escalated situations to ensure client and staff safety
- Vehicle travel to community sites for visits/walk-throughs
- Able to walk up and open doors
