**ACCOUNTABILITY OBJECTIVE**

Are you passionate about making a difference in the lives of seniors? Our Move-in Coordinator position is critical to our communities by ensuring that all residents moving into our residences are provided the best possible care while providing a comfortable, safe and nurturing environment. Under the direction of the General Manager, the Move-in Coordinator is responsible for pre-move in assessments, development of the plan of care and providing nursing support to our new residents.

**BACKGROUND/QUALIFICATIONS**:

- Minimum grade 12 education required.
- Has obtained a recognized nursing license (RPN/LPN licensing) and in good standing with College of Nurses.
- Certified in Standard First Aid, CPR and AED. - Previous experience working as an RPN/LPN in a health care setting is considered an asset.
- Ability to communicate effectively, both written and verbally, in the English language, nurturing excellent relationships with residents, families and other team members.
- Ability to work with mínimal supervision and to evaluate and meet resident care needs, demonstrating initiative and problem-solving skills.
- Dependable, responsible and an ability to work independently and work wellwith others in a team environment
- Demonstrates a positive attitude necessary to meet the goals and standards of the residence.
- A resident-oriented individual, whose visions and values align with those of the residence.
- Has demonstrated excellent leadership skills.
- Must have proficient computer skills (i.e. Word and Excel).

**KEY RESPONSIBILITIES**
- Participates in and adheres to continuous quality improvement standards.
- Completes pre-move-in assessments, and develops plans of care for all new resident prospects and makes recommendations to the appropriate departments/parties/General Manager, as necessary, regarding an individual’s suitability for residence and their needs.
- Is up-to-date on services provided and works with residents and their families to meet the residents’ needs.
- Conducts assessments to determine the needs of individual residents and organizes care plan to promote maximum resident independence.
- Provides assessments and creates plan of care for residents to promote resident independence. Ensures compliance with the RHRA regulations.
- Ensures the resident’s chart is established, with proper documentation.
- Co-ordinates move-ins and ensures all move in paperwork is completed, including pharmacy admission forms if mediation administration is required.
- Assists as required with post move-in visits and initiates move in checklist.
- Performs other duties as assigned by General Manager or delegate.
- Complies with all corporate policies and procedures.
- Completes all required mandatory training/education.
- Complies with the Occupational Health and Safety Act, Workplace Safety and Insurance Act, and Workplace Hazardous Material Information System/Globally Harmonized System. This includes recognizing health and safety hazards, reporting incidents, fulfilling responsibilities under the applicable legislation, as well as participating in in-services and fire drills. Participates in the ongoing Fire Prevention, and Pest Control programs and any other programs as required.
