Milton, Canada Prime Care Family Health Team Full time

**Prime Care Family Health Team (PCFHT) is currently recruiting for a part time Administrative Care Coordinator to provide mental health care coordination. The Administrative Care Coordinator will be responsible for planning the coordination of services to patients, their families, and caregivers. Working in collaboration with an interdisciplinary team, the Administrative Care Coordinator will share expertise and knowledge of resources for mental health treatment in the community and within PCFHT, as well as services for mental health promotion and disease prevention. They will be the first point of contact for patients within PCFHT referred for mental health related concerns and will facilitate connection of the patient to the most appropriate service, as well as screening for patients in need of expedited care or urgent further assessment.**

**Roles and Responsibilities**:
**Assessment**:

- **Review client clinical information obtained through the physician or IHP referral, laboratory results and health records, requesting further information if needed**
- **Conduct assessments with patients/families to obtain relevant information and assess the patient’s needs**
- **Assess for imminent safety concerns, and collaborate with appropriate team members as needed**
- **Work with community networks, the PCFHT team, and referring physician to facilitate patient’s timely access to appropriate resources**
- **Utilize virtual platforms to provide effective delivery of care**
- **Provision of care coordination and system navigation for patients**
- **Work with primary care physicians (PCP) to enhance knowledge and connections to available community-based support for patients**
- **Be available for patient’s to reach out to if initial treatment plans do not meet therapeutic needs or if the patient has difficulty connecting with the recommended resource**

**Planning**:

- **In collaboration with a non-affiliated primary care provider (PCP) and other members of the FHT interdisciplinary team, develop and discuss an appropriate individualized care plan with the patient/family/caregiver based on best practice**
- **Provide ongoing case management for patients identified as at risk and enrolled in mental health programs**
- **Collaborate with primary care providers to deliver effective primary care**
- **Recommend treatment protocols to patients and care providers and provide education to patients and team members as required**
- **Document all health notes into the EMR daily and arrange necessary follow-ups.**

**Team Development**:

- **Work effectively within an interdisciplinary team and non-affiliated primary care providers (PCP)**
- **Promote awareness of the FHT’s program and services to care providers and patients**
- **Actively participate in staff, team and committee meetings**
- **Participate in the development, planning, and evaluation of individual and group treatment, education, counseling and health promotion activities of the FHT**
- **Liaise with team members in facilitating appropriate in-house and external education sessions using virtual platforms**
- **Liaise with community partners to optimize the utilization of existing services**
- **Maintain and develop professional competence through ongoing professional development**
- **Stay current and aware of opportunities to implement new, evidence-based methods of client assessment, treatment and programming**
- **Participate in clinical projects as required**

**Skills Knowledge Requirements**:

- **Team player demonstrated the ability to engage in truly collaborative practice with Inter-professional colleagues both across the FHT, multidisciplinary members and the physician office teams**
- **Strong communication and interpersonal skills, with ability to balance patient advocacy with boundaries in complex interactions**
- **Leadership, organization, evaluation, time management, and initiative**
- **Proficiency in the use of computer hardware and software, particularly in Microsoft Word, Excel, Outlook and electronic medical records**
- **Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the PCFHT**
- **Other duties as required**

**Education/Specific Job Requirements**
- **Police Record/Vulnerable Sector Check**

**Experience**:

- **Minimum 2 to 3 years in Health Care Administration and Patient Care Coordination**:

- **Minimum 2 to 3 years experience in a Community and/or Primary Health Care setting with a focus on mental Health care**
