**About the role**:
Join SRx Health Solutions team as a Biologic Care Coordinator. We are seeking a dedicated individual to provide vital patient support, advocate for patient needs, and ensure seamless coordination of care for biologic medications and therapies. You will work in close collaboration with prescribing physicians, office staff, patients and other healthcare stakeholders in the circle of care to deliver high quality healthcare services.

**What you will be doing**:

- Patient Support and Advocacy:Provide support throughout the healthcare journey.

Advocate for patients' needs, ensuring access to resources and therapies.
- Coordination of Care:Collaborate with SRx Health pharmacies, physicians, and healthcare providers.

Develop and implement individualized care plans.
- Education and Support:Provide high-level education on medical conditions and treatment options.

Empower patients to actively participate in healthcare decisions.
- Documentation:Maintain accurate patient records and communicate progress to the healthcare team.
- Compliance and Quality Assurance:Ensure care meets regulatory and quality assurance standards.

Comply with PIPEDA, applicable provincial legislation, and payer regulations.
Act with high ethical standards and in compliance with all SRx Health Solutions internal policies and regulation.

**Additional Responsibilities**:

- Assist the prescribing physician in completing patient enrollments.
- Liaise and advocate with third-party providers and provincial governments.
- Review patient charts (paper and electronic-based).
- Schedule patients and facilitate communication with healthcare professionals.
- Act as a central resource liaison for program stakeholders.
- Ensure accurate patient records for program reporting.

**Qualifications**:

- Education: Certificate in a healthcare-related field (pharmacy assistant or technician education acceptable).
- Clinical Background: Previous experience in a clinical setting considered an asset.
- Knowledge of Biologics: Familiarity with biologic medications and therapies an asset.
- Communication Skills: Strong verbal and written communication skills.
- Organizational Skills: Excellent organizational skills for managing patient care.
- Patient Advocacy: Understanding of patient advocacy principles.
- Insurance Knowledge: Familiarity with health insurance processes and requirements.
- Interpersonal Skills: Ability to work well with diverse individuals.
- Problem-Solving Skills: Ability to identify and solve problems efficiently.
- Computer Skills: Proficiency in using healthcare information systems and electronic medical records.
- Regulatory Compliance: Understanding and adherence to relevant healthcare regulations and compliance standards.
