**Position Overview**:
Our Community Care Coordinators are at the heart of Skipping Stone’s programs. They are often the first individual someone meets with and act as the conduit to get clients connected with the various supports and services to meet their unique needs. As part of our community empowerment model, our Community Care Coordinators are individuals who first and foremost have shared lived experience with our clients who can connect with people where they are at.
The Community Care Coordinator is a:
- individual with shared lived experience to the communities we serve
- skilled listener and advocate
- person with strong boundaries and self-care practices
- clear, kind and confident communicator both verbally and in writing
- person who practices the principles of intersectional anti-racism, anti-oppression and trauma informed care and adheres to Skipping Stone’s values.
The role consists of:
- Meeting clients virtually and in person at Skipping Stone’s downtown offices on a set weekly schedule with the expectation of delivering approx. 16 appointments per week.
- Performing new client intake appointments which includes a comprehensive needs assessment and community service referral provision
- Maintaining good working knowledge of available services and supports for trans and gender diverse individuals and their families
- Carrying out peer to peer Community Coaching and Gender Gear appointments following an empowerment-focused goal-setting framework
- Clear, confidential and professional documentation of all client interactions using provided templates and guidelines
- Supporting with occasional administrative, project or event based agency needs as required

Skipping Stone operates on the lands of the Niitsitapi people, including the Siksika, Piikani, Kainai, as well as the people of the Iyaxe Nakoda and Tsuut’ina first nations. These lands are also within districts 5 & 6 of the Battle River Territory of the Metis Nation of Alberta.
