**We are currently recruiting for a Care Coordinator**
**Job Summary**:
Reporting to the Director of Nursing, the Care Coordinator is a Licensed Practical Nurse and is responsible to provide scheduled and unscheduled, resident-oriented, goal-directed nursing care and related services in a Trico LivingWell community.
The Care Coordinator adheres to all related provincial/local acts, standards, regulatory health authorities, Trico LivingWell standards, guidelines, policies and procedures and WELL Standards. The Care Coordinator must abide by their regulatory body’s Standards of Practice and Code of Ethics.

**Key Responsibilities**:
- Works as a member of the inter-professional team by maintaining effective communication and good interpersonal relationships with medical and allied health care members, residents, families, employees and other departments;
- Acts as a resource for questions and concerns regarding practices or health care issues;
- Completes and ensures resident’s care assessments, personalized health care plans, and documentation are accurate, up-to-date with changes communicated as required;
- Responds to and manages significant changes in resident’s condition and/or medical emergencies and provides emergency first aid as needed
- Responds to resident, family and visitor concerns and ensures appropriate action is taken within decision making authority, and concern is reported to Director of Nursing, Executive Director and/or other departments;
- Coordinates emergency response in absence of Director of Nursing or Executive Director;
- Provides cognitive, social, emotional, and psychological support to residents and families;
- Participates in and/or leads the IP&C committee, OH&S committee, Fire and Safety Committee, Outbreak & Pandemic Committees, Immunization Clinics;
- Ensures adequate nursing supplies and equipment available and places orders as necessary; ensures efficient and responsible use of such supplies
- Participates in mandatory ongoing training, in-services, and education;
- Participates in strategic planning, ad hoc projects and educational activities as assigned for the organization;
- Participates in the ongoing review of nursing care standards, policies and procedures
- Understands, adheres, and practices of the Health Information Act (HIA), Supportive Living Accommodations Standards (SLAS), Accreditation Standards, WELL Building Standards, Freedom of Information and Protection of Privacy (FOIPP) and Protection of Persons in Care Act (PPCA), and Public Health Act (PHA) and all applicable Acts, Legislations, Regulations, and Standards;
