The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating care for clients referred to our Continuing Care program.
As a Continuing Care Coordinator, you will work closely with clients, families, caregivers, and healthcare teams to identify goals, unmet needs, and resource requirements. You will develop and implement care plans that maximize available services and resources to support clients in their homes and communities.
You will also be responsible for authorizing services, making referrals, and linking clients with other professional and volunteer resources. Your strong assessment, interviewing, and negotiation skills will enable you to effectively communicate with clients, families, and healthcare teams to achieve optimal outcomes.
We are looking for a highly motivated and organized individual with a strong background in healthcare, preferably in a community or long-term care setting. You will possess excellent assessment, interviewing, and negotiation skills, as well as the ability to work independently and collaboratively in a busy multidisciplinary environment.
The ideal candidate will have a Bachelor's degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies, and current registration with the applicable Nova Scotia Professional Regulatory body. You will also have a minimum of three years' experience in a related field, with demonstrated knowledge and/or experience in dedicated case management or discharge planning.
