About the Opportunity

The Nova Scotia Health Authority is seeking a skilled Continuing Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for assessing, planning, and coordinating care for clients referred to Continuing Care. Your expertise will help individuals and families navigate complex health situations and access the resources they need to thrive in their communities.

As a Continuing Care Coordinator, you will work closely with interprofessional teams to facilitate hospital discharge and ensure a smooth transition back to the community. You will also be responsible for authorizing services, making referrals, and linking clients with other professional and volunteer resources. Your goal will be to maximize available services and resources to support clients and families in their homes and communities.

We are looking for a candidate with a strong background in nursing, social work, occupational therapy, physiotherapy, or nutritional studies. You will need to have current and ongoing registration with the applicable Nova Scotia Professional Regulatory body and a minimum of three years' experience in a community or long-term care setting. Your knowledge of community agencies and resources, as well as your excellent assessment, interviewing, and negotiation skills, will be essential in this role.

About You
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy, or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three years' experience in a community or long-term care setting
  • Demonstrated knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding, and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing, and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload, and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset
Hours
  • Permanent Full-time / 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends, and holidays
Compensation and Benefits

$46.75 Hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage, as well as a defined benefit pension plan.
