Job Title: Home Care Team Supervisor

Job Summary: We are seeking a highly skilled and experienced Home Care Team Supervisor to join our team at WRHA Community Health Services - Access Fort Garry. As a key member of our Home Care Team, you will be responsible for supervising and coordinating the delivery of services to Home Care clients, ensuring the efficient use of resources and providing exceptional care to our clients.

About the Role: As a Home Care Team Supervisor, you will be accountable for supervising and ensuring the efficient delivery of services to Home Care clients by coordinating Direct Service Staff (DSS) consisting of ISWs, HCAs, HSWs and RAs in accordance with the HCA/HSW Collective Agreement, program standards and policies, and care plans developed by Case Coordinators (CC).

Key Responsibilities:

  • Supervise and coordinate the delivery of services to Home Care clients
  • Ensure the efficient use of resources and provide exceptional care to clients
  • Recruit, educate and schedule DSS to meet case load demands
  • Provide ongoing supervision and evaluation of DSS
  • Collaborate with Case Coordinators to develop and implement care plans

Requirements:

  • Previous experience supervising staff, preferably in a unionized environment
  • Proficiency in computerized systems (windows environment, word processors, spreadsheets and databases)
  • Previous experience scheduling staff in work assignments
  • Related experience in a healthcare field
  • Recognized post-secondary certificate and/or diploma in a health services or social sciences field
  • Valid driver's license and use of a vehicle

  • Demonstrated ability to function as a team member, work collaboratively across sectors, and be an active participant in multi-disciplinary teams
  • Ability to work independently
