Job Summary

We are seeking a highly skilled and experienced Resource Coordinator to join our team at Pan Am Clinic. As a key member of our Home Care Team, you will be responsible for supervising and coordinating the delivery of services to our clients, ensuring the efficient use of resources and the highest level of care.

Key Responsibilities
  • Supervise and coordinate the work of Direct Service Staff, including ISWs, HCAs, HSWs, and RAs, to ensure the efficient delivery of services to clients.
  • Develop and implement care plans in collaboration with Case Coordinators and other healthcare professionals.
  • Recruit, train, and evaluate staff to ensure they have the necessary skills and knowledge to provide high-quality care.
  • Manage and maintain accurate records and reports, including client information, staff performance, and service delivery data.
  • Collaborate with other healthcare professionals and stakeholders to ensure seamless service delivery and optimal client outcomes.
Requirements
  • Post-secondary certificate or diploma in a health services or social sciences field.
  • Previous experience in a supervisory or management role, preferably in a healthcare setting.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and as part of a team.
  • Knowledge of healthcare policies, procedures, and regulations.
