About the Opportunity

The Care Coordinator is responsible for assessing, planning, authorizing services, and managing the ongoing care of clients referred to Continuing Care. Care coordinators help provide an array of services to assist individuals and families cope with complicated health situations in the community.

Care Coordinators work with interprofessional teams to facilitate hospital discharge to return to the community. They are fiscally responsible for the care, services, and equipment they authorize within available Department of Seniors and Long Term Care policies and programs.

About You
  • Bachelor's Degree in Nursing, Social Work, Occupational Therapy, Physiotherapy or Nutritional Studies
  • Current and ongoing registration with the applicable Nova Scotia Professional Regulatory body
  • Minimum three (3) years' experience with the professional regulatory body
  • Demonstrated knowledge and/or experience in a community or long term care setting
  • Knowledge and/or experience in dedicated case management or discharge planning
  • Competent with computer use, keyboarding and able to conduct assessments using a laptop
  • Knowledge of community agencies and resources
  • Excellent assessment, interviewing and negotiation skills
  • Excellent verbal and written communication skills
  • Ability to work independently and co-operatively in a busy multidisciplinary situation
  • Ability to multi-task, prioritize workload and manage time efficiently
  • Valid driver's license and access to a reliable vehicle
  • Competencies in other languages preferred; French an asset

Hours
  • Long assignment Full-time / 70 Hours Bi-weekly
  • Shifts may include days, evenings, weekends and holidays
Compensation and Benefits

$46.75 Hourly

Successful candidates may be eligible for our benefits package, which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
