Are you an OT/PT/RN who is looking for growth? The ideal candidate will be an Individual with rehab skills looking to lead a team and work with key stakeholders. The individual will manage a team of PSWS, RNs, RPNs, OTs, PTs and home maintenance operators for our Integrated Care programs.


Reporting to our Director of Quality and Integrated Care, the Manager, Integrated Care works within a multi-disciplinary team and is responsible for the oversight and coordination of day-to-day operations surrounding our integrated care clients. Must be a Regulated Health Professional with valid certification in a professional college in the province of Ontario (Registered Nurse, Physical Therapist, or Occupational Therapist preferred). Individuals with allied health and community sector experience would be considered an asset


This position is based out of our head office in Etobicoke and works onsite to support various locations across the GTA. This is a full-time, 12-month contract position with possibility for extension


Shift: Monday - Friday between 8:30am-4:30pm. Rotational weekends are required every third weekend.


KEY RESPONSIBILITIES:

  • Oversees referrals, discharges and caseload management of our THP @ Home Program.
  • Participates in daily care conferences.
  • Works with the THP @ Home Supervisors to manage active caseload of clients and in-home support.
  • Works with sub-contractors as required.
  • Oversee the delivery of client care services including staff education, and orientation.
  • Supports program supervisors.
  • Oversee and ensure quality management for the team, ensuring required outcomes are achieved.
  • Ensure onboarding, performance administration and mentoring of clinical teams are up to standard.
  • Ensure standards of client care and services are consistently achieved.
  • Ensure all MOHLTC requirements are met.
  • Collaborate with CANES team and key stakeholders to identify service gaps.
  • Understands best practice guidelines and approaches to care.
  • Responsible for data collection and monthly program statistics and reporting.
  • Supporting additional integrated care programs with the Director of Quality and Integrated Care as required.
  • Other duties as assigned.


QUALIFICATIONS

  • University degree in a relevant health related discipline.
  • Must be a Regulated Health Professional with valid certification in a professional college in the province of Ontario.
  • Excellent verbal and written communication skills and assessment skills
  • Exercise excellent judgement, flexibility, creativity, and sensitivity to changing situations and needs.
  • Strong attention to detail and organizational skills.
  • Excellent priority management, time management, and problem-solving skills.
  • Ability to work independently with minimal supervision and as part of a team.
  • Knowledge of computer programs i.e. interRAI Community Health Assessment Tool, Screener, Alayacare, EPIC, EMRI, Microsoft Suite products is an asset.
  • Comfortable providing care to clients with Infectious Diseases.
  • Current N95 Mask Fit Test Certificate.
  • Valid First Aid & CPR Certificate and a BLS Certificate (must have both).
  • Proof of current immunizations for Measles, Mumps, Rubella and Varicella (MMRV) proof of negative TB skin test within the last twelve (12) months.
  • Must have at least two (2) doses of the COVID-19 vaccination.
  • Fluency in English (written and spoken) and additional language(s) considered an asset.
  • Valid class G driver’s license and access to reliable vehicle.
  • A relevant combination of qualifications and experience will be considered.
  • Clear Police Reference Check with Vulnerable Sector Screening.
