We are seeking Registered Practical Nurses (RPN) to assume the role of Home Support Supervisor to assist our clients in various care environments.

The Home Support Supervisor is responsible for guiding a team of Personal Support Workers (PSWs) in the field, offering continuous training, mentorship, and performance oversight.

Key Responsibilities

Supervise a team of Personal Support Workers to deliver quality personal care to clients in home environments Travel to clients' residences in community and facility settings Collaborate with multidisciplinary teams in both community and facility environments Evaluate client needs based on contractual timelines, develop care plans, and monitor progress towards goals Manage staff performance, conduct evaluations, and implement necessary disciplinary actions Collect, document, and analyze feedback from clients and staff while monitoring key performance indicators such as caseload volume, staff turnover, satisfaction levels, and performance appraisal completion rates Ensure timely documentation and resolution of all client concerns and complaints Represent Spectrum Health Care by participating in local healthcare or business committees and community events Engage in ongoing professional development through internal and external educational opportunities

Required Qualifications

Current registration with the Colleges of Nurses of Ontario in good standing At least three (3) years of progressive leadership experience as a direct supervisor, preferably in a community setting Strong interpersonal skills with the ability to manage challenging situations consistently and objectively Proficient in report writing and effective verbal and written communication Excellent English communication skills (additional language skills are an asset) Comprehensive understanding of community nursing principles, service delivery to clients, and program development, implementation, and evaluation Valid driver's license and access to a reliable vehicle Basic proficiency in Microsoft Word and Excel, with the ability to learn proprietary software
