Key Qualifications

  • Must be a regulated health professional (RN/RPN) with at least 2 years of experience.
  • Preferably possess 2-3 years of recent managerial experience in a community healthcare setting.
  • Strong understanding of community nursing principles and client service delivery.
  • Ability to lead and supervise a team of Personal Support Workers (PSWs).
  • Valid driver's license and current CPR certification are required.
  • Demonstrated time management skills, professionalism, and confidentiality.
  • Excellent communication skills to build relationships with Home Care Staff and clients.
  • Capacity to work independently while also being an effective team player.
  • Commitment to continuous learning and personal development.
  • Must pass a Police Criminal Record Check with Vulnerable Sector Screening.

Core Responsibilities

  • Foster a culture of compassion, integrity, and quality care within the community.
  • Conduct initial and ongoing assessments for clients requiring care services.
  • Lead and support a team of PSWs to ensure high-quality, client-centered service delivery that aligns with company standards.
  • Develop and maintain care plans for clients, ensuring regular updates as necessary.
  • Facilitate training sessions and educational opportunities for Personal Support Workers.
  • Address and document client concerns and complaints promptly.
  • Stay informed about the Standards of Practice set by the Provincial regulating body.
  • Collaborate closely with Coordinators for the scheduling of Personal Support Workers.
  • Assist in the recruitment, orientation, training, and performance evaluation of Personal Support Workers.
  • Maintain confidentiality regarding client, PSW, and corporate information.
  • Engage in proactive Health & Safety initiatives while performing all responsibilities.
  • Complete additional tasks as required.
