Job Summary
As a member of the PhoenixCentre Mental Health and Substance Use Team, the Program Navigator, at PhoenixSociety participates in the recovery and capacity building of residents tomaximize their social and economic potential and support their goals forsuccessful independent living in the community.

The Phoenix Society is amulti-service agency located in the Lower Mainland of British Columbiadedicated to providing accessible services and opportunities to people who facebarriers related to addiction, mental health, housing, education, criminaljustice involvement and/or employment.
Work Performed
Client Services
Advocate for residents of the PhoenixIntensive Residential Treatment Program (IRT) Discharge planning services for thoseresidents who have participated in IRT programming. ie: housing search in homecommunity, resource and referrals. Ensure transportation for IRT clientsarriving to and leaving from the Phoenix Center at intake and discharge Assist in acquiring personaldocumentation Ensure ongoing assessment of clientprogress regarding levels of support needed Support individuals through crisisincluding tenancy breakdown Provide support for clients to developlife skills and support community engagement Offer assistance for rental applicationsas well as other required documentation such as applying for personalidentification, healthcare requests, training, jobs, etc. Educate and provide consultation toclients, families and healthcare professionals Make effective referrals, reflectingin-depth knowledge of community resources and strong collaborative skills inworking with other community partners Develop, coordinate and deliverpsycho-educational and support groups Ensure that client files are maintainedand current within established guidelines Report to and discuss case concerns withsupervisory positions and the integrated services team Maintain accurate statistics throughoutpractice and provide monthly reports Create and maintain a therapeuticenvironment following therapeutic community principles which encourageawareness of inter-dependence of community members, and the value ofresponsible concern for self and others Act as a positive role model for staffand clients Act as a back-up to outreach or supportstaff as neededOperations
Identify areas for improvement andincreased efficiency in building and program operations, technology, servicesoffered, reporting and reports to supervisory positions Follow and implement universalprecautions and infection control Ensure that program regulations, policiesand procedures are followed Maintain up-to-date knowledge of internalsafety policies and procedures, as well as other emergency procedures Organize and handle client information inaccordance with all privacy legislation Organize and assemble monthlyadministrative reports Maintain agency standards of work qualityCommunity Relations
Develop positive collaborativerelationships with appropriate community partners and external stakeholders Represent the Agency in the communitythrough effective and professional interaction with all stakeholders (e.g., BCHousing, Fraser Health, BC Corrections Branch, etc.), human service and healthservice agencies, government, media, neighbours and the general public Maintain a thorough knowledge of social,economic, recreational and educational resources in the community Attend public and organizational eventsas required Maintain a thorough knowledge of social,economic, recreational, and educational resources in the community Foster teamwork within the PhoenixSociety and with service stakeholders. Take action required to foster teamworkand resolve problems that may ariseQualifications
Post-secondary diploma (e.g., 2 to 3years) in social sciences, counselling, social work, community services, etc. Minimum one (1) year of experienceworking with marginalized populations, substance use services, mental health,corrections, etc., or equivalent Ability to administer Narcan and respondeffectively to overdose. Knowledge of infection control standards and abilityto follow infection control policies Satisfactory completion of a criminalrecords check with vulnerable sector screening is required Satisfactory completion of TuberculosisScreening form is required Satisfactory completion of EmployeeImmunization Record form is required First aid certification with CPR isrequired Valid class 5 driver’s license isrequiredSkills &Knowledge
